Post-Treatment Mentoring

What Is Post-Treatment Mentoring

Our aim is for adolescents and young adults to experience a smooth and long-lasting transition to independent living after therapeutic treatment. Transitioning from a structured treatment environment to functioning in adult life, often requires troubleshooting and upkeep. We can help with maintaining therapeutic gains, the prevention of setbacks, and even helping students bounce back from struggles after transitioning.

What the Research Shows

Did you know that over 60% of students require support and cannot maintain a full level of independence within just the first 9 months after transitioning from a treatment program?

We feel this statistic is FAR too high. This is why we’ve created the Post-Treatment Mentoring program here at Level-Up Life.

Student Success Rates in Transitioning to Independence

Why Choose a Post-Treatment Mentoring Program?

There are many problems we aim to solve, including but not limited to:

  • Saving families thousands of dollars by avoiding the need for readmission to treatment centers

  • Ease tension in families upon graduation

  • Help students build confidence in building their own support network

  • Help students create their own routines, habits, and systems

  • Improve the efficacy of therapeutic interventions

Bridging the Gap Between Treatment and Life After Treatment

As your student starts to work on their growth plan, they will start to see the connection between the following competencies:

Academics - Emotional Regulation - Communication - Sleep Exercise and Nutrition - Social Networks - And More!!

When your student makes these connections and adjusts accordingly, their independence rises and the need for us decreases. This is how we bridge the gap and how they will leave Level-Up Life confident and thriving.

Fully Remote

Our program is fully remote to travel with your student wherever they go. Our hope is to simplify the transition process so you or your student go wherever they need after treatment.

How It Works

Amazing Coaches!

Our coaches are highly trained in helping with the transition process. They are skilled in helping students with understanding the necessity of life skills and with how to implement them.

Independence

Over time, your student will hit certain independence milestones and will need less and less of our service until finally, they can face life’s challenges on their own with confidence.

If you are a professional looking to support the patients in your care, contact us to discuss how our partnerships can help make care more accessible for families.
